CHILD DROP-OFF LIABILITY WAIVER AND CONSENT FORM
1. Acknowledgment of SupervisionI understand and agree that this is a supervised environment but not a one-on-one childcare service. Staff members are responsible for general supervision of multiple children and are not individually assigned to my child.
2. Assumption of Risk
I acknowledge that participation in recreational activities involves inherent risks, including but not limited to falls, collisions, and injuries. I voluntarily assume all such risks on behalf of my child.

I confirm that my child is physically able to participate in the activities and does not have any medical condition that would prevent safe participation, unless disclosed below:

I hereby release, waive, and discharge the facility, its owners, employees, and affiliates from any and all liability for injuries, damages, or losses arising from my child’s participation, except in cases of gross negligence or willful misconduct.
5. Emergency Authorization
In the event of an emergency, I authorize the staff to seek medical treatment for my child if I cannot be reached.
6. Pick-Up Policy
I understand that I must return on time to pick up my child and that additional fees may apply for late pick-up.
7. Behavior Policy
I acknowledge that inappropriate or unsafe behavior may result in my child being removed from the activity area, and I may be contacted for immediate pick-up.
